Who is the service for?
Social rehabilitation is intended for you if you are an 18 to 65-year-old resident of the Western Uusimaa region and would like to find ways to strengthen your mental and physical resources and expand your living environment. The aim of social rehabilitation is to strengthen your social functioning and support your participation. We will plan the implementation of social rehabilitation with you based on your goals.
In social rehabilitation, you will work with a social instructor who will support you individually towards your goals. The rehabilitation path can also make use of group activities. Social rehabilitation groups are rehabilitation groups that aim to strengthen your everyday skills and social skills and support your participation, among other things.
Social rehabilitation is a social service organised in accordance with the Social Welfare Act, and participation in it is voluntary and free of charge.
The service is suitable for different life situations, such as if you are unemployed and searching for a job, on rehabilitation subsidy or looking for a new direction in life. The key is your own desire to seek change and your ability to commit to regular activities outside your home. You cannot participate in the service during an acute crisis.

How can I participate in the service?
You cannot register for social rehabilitation services directly. Instead, the decision to grant the services is based on a service needs assessment carried out with you by a social services professional at Services Supporting Employment.

Conditions for participation:
- The important thing with regard to your participation is your own wish to undergo rehabilitation and your ability to commit to regular meetings.
- We will agree on the meeting schedule with you at the start of your social rehabilitation. You will meet with the social instructor up to once a week.
- You must inform your social instructor at social rehabilitation services if you are unable to come to a meeting.

Conditions for participating in the service:
Our service is targeted at 18–65-year-old residents of Western Uusimaa who have been identified as needing social rehabilitation as a result of a service needs assessment.
You cannot register for social rehabilitation services directly. Instead, the decision to grant the services is based on a service needs assessment carried out with you by a social services professional at Services Supporting Employment, which are part of social services for adults.
The social rehabilitation service is suitable for various life situations. For example, you can participate in the service if you are an unemployed jobseeker, on rehabilitation subsidy, on sick leave, on disability pension, etc. You are required to have a sufficient level of Finnish proficiency to participate in the service, as we provide the social rehabilitation service in Finnish. Participation also requires you to have the desire to improve your condition and seek change.

- You will be able to strengthen your participation, social functioning, and mental and physical resources.
- You will receive information on different services and rehabilitation opportunities.

- You will identify suitable further paths for yourself together with your social instructor.
We do not use a consent form, but our service is voluntary, and we must always establish the client’s own desire and ability to participate in the rehabilitation. The service can only be granted based on a service needs assessment carried out with the client by a social services professional, in which we will agree in more detail on the goals of the social rehabilitation and the support to be provided.
How is the service organised?
The social rehabilitation team operates throughout the entire Western Uusimaa region.
The core elements of the social rehabilitation team’s work with clients are working with clients individually and group activities.
We will assign you a personal social rehabilitation social instructor for the duration of your rehabilitation, and you will work with them to reach your goals. We will plan the social rehabilitation service based on your goals. This means that the duration of the rehabilitation depends on your situation, but the maximum duration is 18 months.
Your designated worker at Services Supporting Employment will coordinate your service package. During the rehabilitation, you will meet up as a network and assess the situation together.
We will provide the social rehabilitation primarily in the form of face-to-face meetings. In between meetings, you can reach your social rehabilitation social instructor by phone if necessary.
The social rehabilitation social instructors approach the work with a coaching mentality and work in the field. They can work with you in different environments as needed to expand your living environment and help you practise engaging with different environments.
The social rehabilitation group activities provide activities and social contacts that support rehabilitation.
The purpose of our service is to strengthen your social functioning, mental and physical resources, and participation. During the rehabilitation, you will work on your goals at your own pace, receive information about different rehabilitation opportunities, and consider with your social instructor what would be the right path for you going forward.
Client meetings with the social instructor will be held up to once a week, usually at the beginning of the service, and they will become less frequent as your rehabilitation progresses. The social rehabilitation groups will also meet once a week.
Our service is free of charge and voluntary. The important thing is that you have the desire to improve your condition and the ability to commit to systematic work.
In social rehabilitation, we do not provide support at your home; instead, our service always seeks to bring you out of your home.
